Back pain is a very common condition, four out of five people have experienced it at some point in their lives. Most commonly occurs in the lower part of the back, called the Lumbar Spine.

SYMPTOMS OF BACK PAIN

Symptoms of back pain can come on suddenly or appear gradually. Sometimes, pain occurs after a specific event, such as bending to pick something up. Other times, you may not know what caused the pain. Pain may be sharp or dull and achy, and it may radiate to your bottom or down the back of your legs (sciatica). Pain is often worse in certain positions (like bending over or standing or sitting) and probably gets better when you lie down. Pain may be constant or intermittent, it varies from person to person.

There may be other symptoms like muscle spasms and a feeling of stiffness/tightness or heaviness in the back.

CAUSES OF BACK PAIN

Mechanical

Most commonly this is due to injury to the spine, intervertebral discs, or soft tissues. Could be due to lifting heavy or awkward objects incorrectly, lack of exercise, having a physically demanding job, or being stressed/depressed.

Degenerative

Osteoarthritis of the spine includes facet joint osteoarthritis, sacroiliac joint osteoarthritis, spinal stenosis, and degenerative disc disease. Furthermore, osteoporotic compressive fractures are also a degenerative process.

Inflammatory

This is caused primarily due to inflammatory (seronegative) spondyloarthropathies such as ankylosing spondylitis, Rheumatoid Arthritis. Sacroiliitis is most commonly seen.

Oncologic

could arise due to cancer in the spine/adjacent areas

Infectious

Infections of the spine, discs, epidural abscesses, or muscular/soft tissue abscesses

Arthritis

Osteoarthritis can affect the lower back. In some cases, arthritis in the spine can lead to a narrowing of the space around the spinal cord, a condition called spinal stenosis.

DIAGNOSIS OF back PAIN

The doctor will ask various questions regarding the pain/condition and carry out a physical assessment/examination to find out what is going on. Additionally, medical imaging like XRAYs and MRIs, and blood tests may be required to get evidence of underlying pathology.
